Case Manager - care planning (remote)

Fernwood Glen Health is looking for a Case Manager. The job centres on care planning for electronic records, and it would suit someone who likes steady, practical work. You would work closely with colleagues in operations, support and management. What you will do • Connect clients with partner agencies. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for electronic records. • Meet clients, understand their needs and build a simple plan. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with electronic records. Working arrangement • Part-time, about 28 hours a week, with flexible days. • Remote-friendly, with an occasional day on site in Colorado Springs. • Applications are reviewed on a rolling basis.
