Case Manager - care planning (remote)

We are growing our team at Foxglove Junction Community Partners and adding a Case Manager whose main responsibility will be care planning. Mission matters here, and so does looking after staff. What you will do • Connect clients with partner agencies. • Help with applications and appointments for benefits enrolment. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with benefits enrolment. Working arrangement • Contract for twelve months, with the possibility of becoming permanent. • Fully remote, with a monthly team day in Colorado Springs. • We will tell you what to expect at each step of the hiring process.
