Case Manager - care planning (remote)

Greenridge Thistle Early Learning Group is looking for a Case Manager. The job centres on care planning for meal programmes, and it would suit someone who likes steady, practical work. You would join a small organisation of about 134 people, working mostly from home. What you will do • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for meal programmes. • Connect clients with partner agencies. • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Experience with meal programmes. • Bilingual English and Spanish. Working arrangement • Part-time, about 20 hours a week, spread across three days. • Fully remote, with quarterly visits in Jackson. • Applications are reviewed on a rolling basis.
