Case Manager - care planning (remote)

We are growing our team at Harborlight Sparrow Care Services and adding a Case Manager whose main responsibility will be care planning. You would work closely with colleagues in operations, support and management. What you will do • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Help with applications and appointments for care plans. • Follow up regularly and update records accurately. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with care plans. Working arrangement • Part-time, about 16 hours a week, afternoons. • Remote-friendly, with quarterly visits in Knoxville. • Applications are reviewed on a rolling basis.
