Case Manager - care planning (remote)

We are growing our team at Bluepeak Beacon Community Partners and adding a Case Manager whose main responsibility will be care planning. You would join a large organisation of more than 1,000 people, working mostly from home. What you will do • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for food distribution. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with food distribution. Working arrangement • Full-time, on a regular day shift. • Fully remote, with a monthly team day in Baltimore. • Applications are reviewed on a rolling basis.
