Case Manager - care planning (remote)

Summit Ember Housing is looking for a Case Manager. The job centres on care planning for repair requests, and it would suit someone who likes steady, practical work. Collaboration with local agencies is constant. What you will do • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Help with applications and appointments for repair requests. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with repair requests. Working arrangement • Contract for eight months, with clear deliverables from the first week. • Remote-friendly, with a monthly team day in Houston. • Applications are reviewed on a rolling basis.
