Case Manager - care planning (remote)

Summit Ivory Health Partners provides primary and specialty care in Madison, and we need a Case Manager to lead our care planning. You would work closely with colleagues in operations, support and management. What you will do • Help with applications and appointments for electronic records. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Experience with electronic records. • Bilingual English and Spanish. Working arrangement • Full-time, with a steady weekday schedule. • Fully remote, with a monthly team day in Madison. • Applications are reviewed on a rolling basis.
