Case Manager - care planning (remote)

Lumen Cobalt Early Learning operates neighbourhood childcare around Des Moines. We are hiring a Case Manager to take on care planning. You would work closely with colleagues in operations, support and management. What you will do • Help with applications and appointments for licensing records. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with licensing records. Working arrangement • Full-time, on a regular day shift. • Fully remote, with an occasional day on site in Des Moines. • We will tell you what to expect at each step of the hiring process.
