Case Manager - care planning (remote)

Fernwood Vista Community Partners is looking for a Case Manager. The job centres on care planning for food distribution, and it would suit someone who likes steady, practical work. You would work closely with colleagues in operations, support and management. What you will do • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Help with applications and appointments for food distribution. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with food distribution.
