Case Manager - care planning (remote)

Nova Summit Property Management is looking for a Case Manager. The job centres on care planning for resident events, and it would suit someone who likes steady, practical work. You would work closely with colleagues in operations, support and management. What you will do • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for resident events. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Experience with resident events. • Bilingual English and Spanish. Working arrangement • Contract for four months, with clear deliverables from the first week. • Fully remote, with an occasional day on site in Rochester. • We reply to every applicant.
