Case Manager - care planning (remote)

We are growing our team at Rivermark Orchard Care Services and adding a Case Manager whose main responsibility will be care planning. You would join a small organisation of about 33 people, working mostly from home. What you will do • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Help with applications and appointments for caregiver training. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with caregiver training. Working arrangement • Part-time, about 24 hours a week, afternoons. • Fully remote, with a monthly team day in Las Vegas. • Applications are reviewed on a rolling basis.
