Case Manager - care planning (remote)

Westbrook Ember Housing is a nonprofit housing provider in Minneapolis. We are hiring a Case Manager to take on care planning. Housing is our mission and residents are our neighbours. What you will do • Help with applications and appointments for rent assistance. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. • Connect clients with partner agencies. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with rent assistance. Working arrangement • Part-time, about 24 hours a week, mornings. • Fully remote, with an occasional day on site in Minneapolis. • We will tell you what to expect at each step of the hiring process.
