Case Manager - care planning (Raleigh, NC)

Harborlight Sparrow Care Services supports families and individuals in Raleigh with personal care and coordination, and we need a Case Manager to lead our care planning. We train and promote from within. What you will do • Follow up regularly and update records accurately. • Help with applications and appointments for licensing. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with licensing. Working arrangement • Full-time, with a steady weekday schedule. • On site in Raleigh. • We will tell you what to expect at each step of the hiring process.
