Case Manager - care planning (Raleigh, NC)

Lumen Health Group runs clinics and patient services around Raleigh, and we need a Case Manager to lead our care planning. Clinicians and administrators work side by side. What you will do • Connect clients with partner agencies. • Follow up regularly and update records accurately. • Help with applications and appointments for insurance authorisations. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Experience with insurance authorisations. • Bilingual English and Spanish. Working arrangement • Part-time, about 16 hours a week, with flexible days. • On site in Raleigh. • We will tell you what to expect at each step of the hiring process.
