Case Manager - care planning (Raleigh, NC)

Clearpath Fox Learning Group is looking for a Case Manager. The job centres on care planning for family communication, and it would suit someone who likes steady, practical work. Collaboration between teachers and staff is strong. What you will do • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Help with applications and appointments for family communication. • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with family communication.
