Case Manager - care planning (Raleigh, NC)

We are growing our team at Ironwood Basin Care Services and adding a Case Manager whose main responsibility will be care planning. You would work closely with colleagues in operations, support and management. What you will do • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for care plans. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with care plans. Working arrangement • Part-time, about 20 hours a week, afternoons. • On site in Raleigh. • Applications are reviewed on a rolling basis.
