Case Manager - care planning (Raleigh, NC)

Redstone Summit Learning Group runs classrooms and tutoring across the Raleigh area, and we need a Case Manager to lead our care planning. You would work closely with colleagues in operations, support and management. What you will do • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Help with applications and appointments for student accounts. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Experience with student accounts. • Bilingual English and Spanish. Working arrangement • Contract for four months, with a review at the end and a chance of extension. • On site in Raleigh. • Applications are reviewed on a rolling basis.
