Case Manager - care planning (Raleigh, NC)

Bluepeak Nest Housing Group provides affordable homes and support services around Raleigh. We are hiring a Case Manager to take on care planning. You would join a small organisation of about 42 people based in Raleigh. What you will do • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. • Help with applications and appointments for maintenance stores. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Experience with maintenance stores. • Bilingual English and Spanish. Working arrangement • Full-time, on a regular day shift. • On site in Raleigh, with some flexibility after the first few months. • We will tell you what to expect at each step of the hiring process.
