Case Manager - care planning (Providence, RI)

Fernwood Lantern Community Partners Group is a Providence charity working with residents on basic needs and opportunity, and we need a Case Manager to lead our care planning. You would work closely with colleagues in operations, support and management. What you will do • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for client intake. • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. • Connect clients with partner agencies. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with client intake. Working arrangement • Full-time, Monday to Friday. • On site in Providence, with some flexibility after the first few months. • Applications are reviewed on a rolling basis.
