Case Manager - care planning (Providence, RI)

Foxglove Summit Learning Group is an education provider in Providence serving students and families. We are hiring a Case Manager to take on care planning. Collaboration between teachers and staff is strong. What you will do • Help with applications and appointments for attendance records. • Connect clients with partner agencies. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Experience with attendance records. • Bilingual English and Spanish. Working arrangement • Full-time, Monday to Friday. • On site in Providence. • We will tell you what to expect at each step of the hiring process.
