Case Manager - care planning (Providence, RI)

Clearpath Quarry Care Services is a social-care provider serving older adults and people with disabilities around Providence. We are hiring a Case Manager to take on care planning. You would work closely with colleagues in operations, support and management. What you will do • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for care plans. • Connect clients with partner agencies. • Follow up regularly and update records accurately. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with care plans. Working arrangement • Part-time, about 28 hours a week, mornings. • On site in Providence. • We will tell you what to expect at each step of the hiring process.
