Case Manager - care planning (Providence, RI)

Lumen Kestrel Community Partners runs community programmes across Providence, and we need a Case Manager to lead our care planning. You would join a small organisation of about 31 people based in Providence. What you will do • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Help with applications and appointments for benefits enrolment. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with benefits enrolment. Working arrangement • Contract for eight months, with the possibility of becoming permanent. • On site in Providence, with some flexibility after the first few months. • Applications are reviewed on a rolling basis.
