Case Manager - care planning (Providence, RI)

Silverline Nest Community Partners is looking for a Case Manager. The job centres on care planning for volunteer shifts, and it would suit someone who likes steady, practical work. What you will do • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for volunteer shifts. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with volunteer shifts. Working arrangement • Contract for four months, with a review at the end and a chance of extension. • On site in Providence, with some flexibility after the first few months. • We will tell you what to expect at each step of the hiring process.
