Case Manager - care planning (Providence, RI)

Greenridge Wilder Learning Group runs classrooms and tutoring across the Providence area. We are hiring a Case Manager to take on care planning. Collaboration between teachers and staff is strong. What you will do • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for enrolment. • Connect clients with partner agencies. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with enrolment. Working arrangement • Part-time, about 24 hours a week, mornings. • On site in Providence, with some flexibility after the first few months. • We will tell you what to expect at each step of the hiring process.
