Case Manager - care planning (Providence, RI)

Clearpath Yarrow Civic Services supports public agencies in Providence with budgeting, records and programmes. We are hiring a Case Manager to take on care planning. We follow procedures carefully and explain them clearly. What you will do • Help with applications and appointments for public records. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with public records. Working arrangement • Full-time, on a regular day shift. • On site in Providence, with some flexibility after the first few months. • Applications are reviewed on a rolling basis.
