Case Manager - care planning (Providence, RI)

Foxglove Summit Learning Group operates schools and learning programmes around Providence. We are hiring a Case Manager to take on care planning. Families are partners. What you will do • Connect clients with partner agencies. • Help with applications and appointments for attendance records. • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with attendance records. Working arrangement • Contract for six months, with clear deliverables from the first week. • On site in Providence. • We reply to every applicant.
