Case Manager - care planning (Portland, OR)

We are growing our team at Clearpath Bright Community Partners Group and adding a Case Manager whose main responsibility will be care planning. We are small enough to know everyone's name. What you will do • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for grant reporting. • Follow up regularly and update records accurately. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Experience with grant reporting. • Bilingual English and Spanish. Working arrangement • Part-time, about 16 hours a week, with flexible days. • On site in Portland, with some flexibility after the first few months. • We reply to every applicant.
