Case Manager - care planning (Portland, OR)

Amberfield Fox Learning Group is an education provider in Portland serving students and families. We are hiring a Case Manager to take on care planning. Collaboration between teachers and staff is strong. What you will do • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for attendance records. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Follow up regularly and update records accurately. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Experience with attendance records. • Bilingual English and Spanish. Working arrangement • Full-time, Monday to Friday. • On site in Portland, with some flexibility after the first few months. • We will tell you what to expect at each step of the hiring process.
