Case Manager - care planning (Portland, OR)

We are growing our team at Lumen Wilder Early Learning Partners and adding a Case Manager whose main responsibility will be care planning. You would join a small organisation of about 72 people based in Portland. What you will do • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Help with applications and appointments for classroom supplies. • Follow up regularly and update records accurately. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with classroom supplies. Working arrangement • Part-time, about 20 hours a week, spread across three days. • On site in Portland. • We will tell you what to expect at each step of the hiring process.
