Case Manager - care planning (Portland, OR)

We are growing our team at Northwind Field Housing and adding a Case Manager whose main responsibility will be care planning. Housing is our mission and residents are our neighbours. What you will do • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for rent assistance. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Experience with rent assistance. • Bilingual English and Spanish. Working arrangement • Contract for twelve months, with clear deliverables from the first week. • On site in Portland, with some flexibility after the first few months. • We will tell you what to expect at each step of the hiring process.
