Case Manager - care planning (Pittsburgh, PA)

We are growing our team at Westbrook Vista Property Management and adding a Case Manager whose main responsibility will be care planning. Maintenance and leasing staff share information every day. What you will do • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for rent collection. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with rent collection. Working arrangement • Contract for four months, with a review at the end and a chance of extension. • On site in Pittsburgh. • We reply to every applicant.
