Case Manager - care planning (Pittsburgh, PA)

Northwind Willow Community Partners is looking for a Case Manager. The job centres on care planning for volunteer shifts, and it would suit someone who likes steady, practical work. Flexibility and kindness are expected. What you will do • Follow up regularly and update records accurately. • Help with applications and appointments for volunteer shifts. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Experience with volunteer shifts. • Bilingual English and Spanish. Working arrangement • Part-time, about 16 hours a week, mornings. • On site in Pittsburgh, with some flexibility after the first few months. • We will tell you what to expect at each step of the hiring process.
