Case Manager - care planning (Pittsburgh, PA)

We are growing our team at Silverline Vista Community Partners Group and adding a Case Manager whose main responsibility will be care planning. Flexibility and kindness are expected. What you will do • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for donor records. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with donor records. Working arrangement • Full-time, Monday to Friday. • On site in Pittsburgh. • We reply to every applicant.
