Case Manager - care planning (Pittsburgh, PA)

Highbridge Mill Health runs clinics and patient services around Pittsburgh. We are hiring a Case Manager to take on care planning. You would join a large organisation of about 601 people based in Pittsburgh. What you will do • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Help with applications and appointments for electronic records. • Meet clients, understand their needs and build a simple plan. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Experience with electronic records. • Bilingual English and Spanish. Working arrangement • Full-time, on a regular day shift. • On site in Pittsburgh. • We reply to every applicant.
