Case Manager - care planning (Pittsburgh, PA)

Westbrook Clear Care Services Group provides in-home and community support in Pittsburgh, and we need a Case Manager to lead our care planning. We train and promote from within. What you will do • Connect clients with partner agencies. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for visit scheduling. • Meet clients, understand their needs and build a simple plan. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Experience with visit scheduling. • Bilingual English and Spanish. Working arrangement • Contract for eight months, with the possibility of becoming permanent. • On site in Pittsburgh. • Applications are reviewed on a rolling basis.
