Case Manager - care planning (Pittsburgh, PA)

Summit Ivory Health runs clinics and patient services around Pittsburgh, and we need a Case Manager to lead our care planning. You would work closely with colleagues in operations, support and management. What you will do • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Help with applications and appointments for insurance authorisations. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Experience with insurance authorisations. • Bilingual English and Spanish. Working arrangement • Full-time, Monday to Friday. • On site in Pittsburgh, with some flexibility after the first few months. • We reply to every applicant.
