Case Manager - care planning (Pittsburgh, PA)

Stonegate Mill Care Services Group is a social-care provider serving older adults and people with disabilities around Pittsburgh. We are hiring a Case Manager to take on care planning. We train and promote from within. What you will do • Help with applications and appointments for client records. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Follow up regularly and update records accurately. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with client records. Working arrangement • Contract for six months, with a review at the end and a chance of extension. • On site in Pittsburgh, with some flexibility after the first few months. • We will tell you what to expect at each step of the hiring process.
