Case Manager - care planning (Phoenix, AZ)

Ironwood Hollow Health is looking for a Case Manager. The job centres on care planning for insurance authorisations, and it would suit someone who likes steady, practical work. You would work closely with colleagues in operations, support and management. What you will do • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Help with applications and appointments for insurance authorisations. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Experience with insurance authorisations. • Bilingual English and Spanish. Working arrangement • Contract for three months, with the possibility of becoming permanent. • On site in Phoenix, with some flexibility after the first few months. • We will tell you what to expect at each step of the hiring process.
