Case Manager - care planning (Phoenix, AZ)

Greenridge Bright Housing provides affordable homes and support services around Phoenix, and we need a Case Manager to lead our care planning. You would work closely with colleagues in operations, support and management. What you will do • Connect clients with partner agencies. • Help with applications and appointments for waiting lists. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with waiting lists. Working arrangement • Part-time, about 20 hours a week, spread across three days. • On site in Phoenix. • We will tell you what to expect at each step of the hiring process.
