Case Manager - care planning (Phoenix, AZ)

Lumen Timber Property Management Group looks after rental homes across the Phoenix area. We are hiring a Case Manager to take on care planning. You would work closely with colleagues in operations, support and management. What you will do • Help with applications and appointments for maintenance requests. • Connect clients with partner agencies. • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with maintenance requests. Working arrangement • Part-time, about 16 hours a week, mornings. • On site in Phoenix, with some flexibility after the first few months. • We will tell you what to expect at each step of the hiring process.
