Case Manager - care planning (Phoenix, AZ)

Stonegate Glen Civic Services provides shared administrative services to local governments around Phoenix, and we need a Case Manager to lead our care planning. We follow procedures carefully and explain them clearly. What you will do • Connect clients with partner agencies. • Follow up regularly and update records accurately. • Help with applications and appointments for public records. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with public records. Working arrangement • Part-time, about 24 hours a week, afternoons. • On site in Phoenix. • Applications are reviewed on a rolling basis.
