Case Manager - care planning (Phoenix, AZ)

Highbridge Haven Learning Group is an education provider in Phoenix serving students and families. We are hiring a Case Manager to take on care planning. You would join a large organisation of more than 1,000 people based in Phoenix. What you will do • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. • Help with applications and appointments for student accounts. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Experience with student accounts. • Bilingual English and Spanish. Working arrangement • Part-time, about 24 hours a week, afternoons. • On site in Phoenix. • We will tell you what to expect at each step of the hiring process.
