Case Manager - care planning (Phoenix, AZ)

Westbrook Stone Care Services provides in-home and community support in Phoenix. We are hiring a Case Manager to take on care planning. We train and promote from within. What you will do • Connect clients with partner agencies. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for visit scheduling. • Meet clients, understand their needs and build a simple plan. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Experience with visit scheduling. • Bilingual English and Spanish. Working arrangement • Full-time, with a steady weekday schedule. • On site in Phoenix. • Applications are reviewed on a rolling basis.
