Case Manager - care planning (Phoenix, AZ)

Bright Kestrel Housing is a nonprofit housing provider in Phoenix, and we need a Case Manager to lead our care planning. Housing is our mission and residents are our neighbours. What you will do • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for maintenance stores. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with maintenance stores. Working arrangement • Contract for nine months, with the possibility of becoming permanent. • On site in Phoenix, with some flexibility after the first few months. • We reply to every applicant.
