Case Manager - care planning (Phoenix, AZ)

Westbrook River Property Management manages apartment communities around Phoenix, and we need a Case Manager to lead our care planning. We are flexible about schedules. What you will do • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Help with applications and appointments for vendor invoices. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with vendor invoices. Working arrangement • Contract for eight months, with clear deliverables from the first week. • On site in Phoenix. • We reply to every applicant.
