Case Manager - care planning (Phoenix, AZ)

Rivermark Field Housing is looking for a Case Manager. The job centres on care planning for lease compliance, and it would suit someone who likes steady, practical work. You would join a large organisation of about 606 people based in Phoenix. What you will do • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Follow up regularly and update records accurately. • Help with applications and appointments for lease compliance. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with lease compliance. Working arrangement • Part-time, about 28 hours a week, afternoons. • On site in Phoenix, with some flexibility after the first few months. • We will tell you what to expect at each step of the hiring process.
