Case Manager - care planning (Phoenix, AZ)

Nova River Property Management Group looks after rental homes across the Phoenix area, and we need a Case Manager to lead our care planning. You would join a large organisation of more than 1,000 people based in Phoenix. What you will do • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for unit turnovers. • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. • Connect clients with partner agencies. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Experience with unit turnovers. • Bilingual English and Spanish. Working arrangement • Full-time, on a regular day shift. • On site in Phoenix, with some flexibility after the first few months. • We will tell you what to expect at each step of the hiring process.
