Case Manager - care planning (Philadelphia, PA)

We are growing our team at Stonegate Fox Property Management and adding a Case Manager whose main responsibility will be care planning. We are flexible about schedules. What you will do • Help with applications and appointments for unit turnovers. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Experience with unit turnovers. • Bilingual English and Spanish. Working arrangement • Contract for twelve months, with clear deliverables from the first week. • On site in Philadelphia. • Applications are reviewed on a rolling basis.
