Case Manager - care planning (Philadelphia, PA)

Westbrook Haven Early Learning is an early-learning provider in Philadelphia, and we need a Case Manager to lead our care planning. You would work closely with colleagues in operations, support and management. What you will do • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Help with applications and appointments for tuition and subsidies. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Experience with tuition and subsidies. • Bilingual English and Spanish.
