Case Manager - care planning (Philadelphia, PA)

Bright Haven Learning Group is an education provider in Philadelphia serving students and families. We are hiring a Case Manager to take on care planning. We follow the school calendar and we like people who like learning. What you will do • Help with applications and appointments for learning materials. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Experience with learning materials. • Bilingual English and Spanish. Working arrangement • Contract for six months, with the possibility of becoming permanent. • On site in Philadelphia. • Applications are reviewed on a rolling basis.
